MHA Offers Community Health Needs Assessment Guidance And Implementation Plan Resources
During Community Health Improvement Week, which is June 8-12 this year, the Missouri Hospital Association is highlighting key community health needs assessment resources available to member hospitals. The most recent CHNA guide, available on MHA’s website, aligns with current IRS requirements while providing practical, step-by-step tools to support an efficient and meaningful CHNA process.
MHA also developed ready-to-use implementation plan templates addressing priority areas such as mental/behavioral health, substance use, chronic conditions, affordable care and support for unhoused populations. Each template includes evidence-based strategies, clear action steps, partner engagement, performance measures and timelines.
Together, these resources help hospitals move from assessment to action — driving measurable improvements in community health.
The Missouri Hospital Association is a nonprofit association in Jefferson City that represents 136 Missouri hospitals. In addition to representation and advocacy on behalf of its membership, the association offers continuing education programs on current health care topics and seeks to educate the public about health care issues.
